Countess of Chester Handover Policy

What is this resource?
This is a policy used by the Countess of Chester Foundation Trust and aims to ensure accurate multi disciplinary handovers out of hours and at weekends, supporting the Hospital at Night team in line with WTD 2009 compliance.

What is in this resource?
The policy contains guidelines of where and when handovers should take place, who should attend and details all the information that should be included in each handover.

Who is this useful for?
This policy is distributed to every member of staff involved in handovers. As a good practice example it is particularly useful for clinical site co-ordinators – as well as all staff attending handovers.
